Internal Medicine- Site Evaluation

Mid Rotation site evaluation: I presented a comprehensive H&P on a patient who presented with back pain for 1 month, but imaging was suggestive of metastatic cancer. I was struggling to present the patient comprehensive story in a concise way. Also, I was also not sure if I should report the ED finding or the finding that I observed during interacting with the patient. Professor Herel helped me to overcome this challenge by suggesting to present study as a case report rather than just an H&P, meaning it should flow like a story from the start point (ED) to the …

Self-Reflection on Internal Medicine Rotation

Skills or situations that are difficult for you (e.g. presentations, focused H&Ps, performing specific types of procedures or specialized interview/pt. education situations) and how you can get better at them I rotated with three different attendings meaning they all had different expectations regarding the morning presentation of the H&P and physically examining the patient. One attending expected a brief description of the chief complaint and HPI, while other attending requested a detailed comprehensive H&P with specific emphasis on HPI and abnormal findings. I personally found both ways of presentation to have drawbacks and strengths. I liked the detailed presentation because …

Final reflection on Clinical Correlations

The Clinician Correlation was overall very effective in bringing all the information I have gathered over the past year and a half. I believe this was a very healthy environment to practice the skills of collecting effective and pertinent history from the patient. I personally struggle to create a working list of differential diagnoses and clinical correlation has helped to progress effectively even though I still struggle with it. Clinical correlation was a training session to work through the puzzle of patient history and come up with a ddx and treatment plan.  I really like the case about TB patients …

Ethics Reflection Essay

This essay is going to highlight the reasons for perusing a Physician Assistant (PA) profession and how I envision the scope of practice as a PA in the future. H&P reflection

What differences do you note between the two H&Ps? Compare to the fist H&P, the second one is more detailed, contains complete physical examinations, 5 differential diagnosis with explanation, plans, and a problem list. Overall, the second one reveals the growth over two semesters as I feel more comfortable with the pertinent ROS and the physical examination. In what ways has your history-taking improved?  Are you eliciting all the important information? My history-taking skill has improved a lot when comparing to my first hospital visit. The first time, I was struggling to ask questions effectively within a limited time period, …
